Downloaded an IPA from app cake, the built in installer could not install it. Went ahead and opened up Filza and it installed properly. Is there something that needs updating for appcake on ios 11.4?

Happens quite often tbh.
I have had one or two (chaos reborn comes to mind) where they install, then removed for low storage tried reinstalling and fails via every install program.
It a pain but ios 11 for you apparently.

If your jailbroken try ext3nder it works well with apps that normal crash on launch, but cant install a few (such as chaos reborn)

same here (ios 11.3, electra 1.1), appcake built-in installation just stopped working suddenly, I even tried on ipas which was in the "appcake downloaded" section (which have installed successfully in the past), but now they hang on infinite "installing". Only installing via Filza helps.

I don't if it is magic or something, but installing Appcake 5.6.0.2 from Xarold repo fixed the problem
